Health Care/Rx/Dental/Disability

FLEXIBLE BENEFITS PACKAGE
Benefit Eligibility Description
Group Health 1st of the month following start date.
  • Two options are available, or employee may opt to waive out of medical coverage.

A Prescription Card 1st of the month following start date.
  • Express Scripts administers the prescription card program. The Rx benefit is offered as part of the following medical plan options:
  • PPO
  • Aetna Health Advantage Plan with HSA
  • Formulary: The plan utilizes a listing of preferred drugs with lower copays.
  • Generic and Branded drugs are covered. When a generic drug is available, it must be dispensed or there will be an additional employee cost.
  • 90-day mail-order service is available for maintenance drugs, cost for a 90 day supply is 2.5 times the copay

Dental Assistance Plan 1st of the month following start date.

Administered by Delta Dental
No cost to employee for single coverage only. Requires contribution for dependent coverage.

  • Plan covers the routine maintenance, diagnostic and preventative services: payment to provider is made at 100% with a Delta dentist based on the U.C.R. schedule for the service.
  • Two routine checkups per year, covered at 100%.
  • Payments for restorative work such as fillings, crowns and extractions are calculated at 75% of the U.C.R. scale for the service.
  • $1,500 per person annual plan year maximum,
  • Limited Orthodontic Services offered to dependents only up to age 19, Lifetime maximum $1,000.
  • The difference between the plan's payment and the dentist's charges is your responsibility.
Long Term Disability Plan 1st of the month following start date.
  • Full-Time Employees provided benefit by Christiana Care at no cost to employee:
  • 50% Salary Replacement - 90 day wait
  • Maximum monthly benefit - $10,000/month (all source)
  • Enhanced options available with additional employee contribution

Blood Bank Membership Immediate.
  • Employees and dependents covered.
  • Annual fee paid by Christiana Care.
  • Provides unlimited supply of blood anywhere in the U.S. at no cost to the employee.
  • Blood is not covered by medical plans.
FSA: Health Care and Dependent Care Accounts 1st of the month following start date.

  • Deposits set aside with before tax dollars for payment of health care and dependent care expenses.
  • Health care account covers medical and/or dental expenses not paid by your insurance.
  • Health care account $5,000 maximum deposit amount.
  • Dependent care account covers child care expenses for children age 13 and under.
  • Dependent care account $5,000 maximum deposit amount.
HSA: Health Savings Account 1st of the month following start date.

  • Aetna Health Advantage Deductible: Single: $1,150 or Family: $2,300
  • CCHS Deductible Contributions: Single $550 or Family $1,100
  • Employee deposits to fund deductible set aside with before tax dollars for payment of medical, dental, Rx and other Medical Care expenses.
  • HSA covers medical, dental and Rx expenses up to deductible limit, including copays and expenses not paid by your insurance.
  • Full Time Employees may contribute to their account up to: Single $2,450 or Family $4,850.
  • Part Time Employees may contribute to their account up to: Single $2,725 or Family $5,400.
  • HSA account is portable; you own the account and all funds in the account.

Paid Time Off

Benefit Eligibility Description
Paid Leave Account After completion of 90 days of service.
  • Pooled time away from work for rest, relaxation, vacation, sickness and other personal reasons
  • 26 to 36 days per year, based on position and length of service.
  • Varies depending on vacation time allotted to specific positions and length of service.
Disability Leave Account After completion of 90 days of service.
  • Paid time away from work for extended illness or leaves of absences
  • 5 days per year with no maximum balance.

Other Programs that support work-life balance include:

  • Flexible work options including - flex time that varies starting time and is available based on departmental function, part time with benefits, part time with limited benefits for 15% higher pay, compressed work weeks with 10 and 12 hour shifts, Weekend Incentive Program that provides higher levels of pay for 3 out of 4 weekends, casual /per diem, job sharing, telecommuting.
  • Leaves of absences that include paternity and personal leaves.

  • Program covers tuition and text books for students in nursing and selected Allied Health programs. Employees may apply as well as others who qualify.

  • No cost to employee, tuition paid directly to school.
  • Employee must qualify to be accepted to program.
  • MSN Degree offered through University of Delaware.

Retirement and Savings

Tax Deferred Annuity Immediate.
  • Employee designates pre-taxed dollars for investment.
  • Contributions made by payroll deduction.

  • Multiple investment options based on asset class, risk and reward, plus 5 LifeSpan Asset Allocation options
  • Programs are administered by Lincoln Financial or Fidelity.
Matching Contribution Program 403 (b) 1 full calendar year of service with a minimum of 1000 service hours per year.
  • Christiana Care matches $.50 for each $1.00 of employee contribution up to match maximum limit.
  • Maximum match limit is based on 6% of annual salary, based on IRS salary maximum.
  • 100% vested after 3 years of service of 1000 hours or more per year.
Defined Contribution Retirement Plan Minimum 1000 hours and 12 months of service.
  • Employer contribution (3% to 5% of earnings) based on length of service. 3 year vesting.
  • Over 15 different investment options based on asset class, risk and reward, plus 5 LifeSpan Asset Allocation options

Additional Benefits

Benefit Eligibility Description
Group Life Insurance Plan 1st of the month following start date.
  • Provided 1 times base pay at no cost to employee or limit benefit to $50,000 available with no imputed income tax

(For individuals with annual salary greater than $50,000)

  • Optional Coverage: employee may choose from 1, 2, or 3 times base pay at additional cost, premium is based age.
  • Maximum insurance up to the lesser of 4 times base salary (includes optional) or $1 million dollars.
  • An insurance amount in excess of $500,000 is subject to Evidence of Insurability (EOI).
Personal Accident Insurance (PAI): 1st of the month following start date.

Employee: 6 coverage options - contributory

As part of the continuing commitment to the well-being of all employees, Christiana Care Health System is very pleased to offer our enhanced Employee Assistance Program (EAP).

To help with work/life balance and provide assistance whenever life gets challenging, the EAP is a confidential resource for assistance with a wide range of concerns and needs, including stress, relationship problems, family matters, financial concerns, emotional distress and many other issues. The benefits are prepaid by Christiana Care and offered at no cost to employees.

This free benefit is provided by CIGNA insurance company. Support is available 24 hours a day, 7 days a week, by phone or on the web. This program provides unlimited telephone consultations with life balance consultants, three free visits with a local EAP provider, web-based tools and online information for you, your family and/or your dependents.

Life balance professional staff consultants can provide one-on-one assistance for a variety of life issues including:

  • Up to 3 face-to-face sessions per issue with a licensed professional EAP counselor close to work or home
  • Coping with stress, anxiety and depression;
  • Caring for an elderly relative or finding senior assistance;
  • Communication difficulties with a manager or co-worker;
  • Raising children and finding child care in your area;
  • Dealing with a family member who has an addiction;
  • Improving your health or controlling a chronic condition;
  • Getting out of debt and managing your finances;
  • Relationship issues, legal issues and much more.
