Who We Are Looking For?
We're actively seeking Behavioral Health Therapists with an interest in a new innovative model of healthcare, specific to the geriatric patient population. The 65+ practice(s) in our Rehoboth and Lewes offices will provide comprehensive medical care to Highmark Medicare Advantage patients. This unique model of care focuses on patient-centered, individualized care plans and standardized clinical outcomes.
Schedule
Monday - Friday, 8am -4:30pm, no weekends, closed on holidays. This position is a hybrid blend of remote/virtual and in person work .

Responsibilities
Conducts individual psychotherapy with assigned patients and group therapy sessions using evidence-based practices.
Conducts psychosocial evaluations to provide diagnostic impressions and treatment recommendations.
Develops coordinated treatment plan in collaboration with other members of the treatment team.
Tracks referrals of patients requesting services.
Ongoing coordination of care with medical providers and psychiatric providers inside and outside the ChristianaCare.
Participates in all assigned programs and attends all continuing education opportunities to meet job requirements and maintain current knowledge of the practice of psychotherapy and social work.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Licensed Clinical Social Worker / LCSW required.
Whether a position can be supported for remote work will be assessed based on whether ChristianaCare is able to meet the business requirements of the proposed remote location.
