As the Hematology/Oncology Clinical Pharmacist, you will provide longitudinal pharmaceutical care to hematology and oncology patients at the Helen F. Graham Cancer Center infusion suite and physician practices by dispensing, overseeing preparation, monitoring, and evaluating of patient specific pharmacotherapy. In this position you will provide extensive direct pharmaceutical care in collaboration with providers in the Cancer Service Line through the infusion suite.  Additionally, you will provide extensive support to the monitoring, operations, and documentation of clinical research protocols within the infusion suite Pharmacy.


  • Review and interpret provider's medication orders, evaluate drug efficacy, therapeutic goals and indications, review for contraindications/precautions, the patient's current status of health, and review drug profiles and patient's medical record for drug-drug, drug-food, drug-laboratory and drug-disease state(s) potentials
  • Coordinates and supervises work-place activities and resources; promotes professional competence; assures that services rendered and work performed are in accordance with specific standards; advises, assists, and supervises work of pharmacy technicians; assumes final responsibility for accuracy of their work; to include sterile product processing in the IV Admixture Service lab and ambulatory chemotherapy areas, where the pharmacist is responsible for assuring USP 800 standards are consistently in compliance
  • Design, implement and monitor patient-specific chemotherapeutic regimens, including supportive care in collaboration with the healthcare team
  • Resolve such problems in consultation with the prescriber and nurse and clarify questioned orders prior to dispensing medication
  • Provide accurate, unbiased, timely, and cost-effective drug information to medical, pharmacy, nursing and other allied health professionals
  • Monitor drug therapy outcomes for targeted patients within established guidelines
  • Demonstrate skill and knowledge necessary to provide pharmaceutical care for oncologic, hematologic and immunologic conditions
  • Manage patients supportive care plan with focus on pain, neuropathy, nausea or vomiting
  • Provide oversight to Pharmacy Services' role with investigational drug studies at the HFGCC
  • Maintain investigational drug records within the CREDIT system and perform routine audits to ensure compliance with investigational drug protocols
  • Collaborate with investigational drug leadership to expedite the onboarding of new protocols within the Pharmacy and assist in onsite drug monitor evaluations of Pharmacy practices
  • Report, record, assess and evaluate adverse drug reactions
  • Identify and submit Safety First Learning Reports (SFLR) and providing follow up assessment
  • Assure handling of medications throughout the hospital and infusion suite complies with state and federal laws and regulations; as well as hospital policies
  • Precept pharmacy residents and students, as assigned
  • Prepare special medications involving compounding procedures and sterile products using aseptic technique when necessary to assure appropriate and timely dispensing
  • Provide pharmacist cross-coverage to maintain pharmacy services, as assigned
  • Support and assist interdepartmental projects as requested (i.e. MUE, P&T monographs, newsletter articles, performance improvement activities, etc)
  • Participate in providing educational sessions (i.e., technician training, inservices, clinical pearls, staff development education) or materials for patient groups and allied health professionals.


  • B.S. in Pharmacy or PharmD degree required
  • ASHP-accredited PGY-1 Pharmacy Residency required
  • Hospital or Outpatient Infusion Experience preferred
  • Current licensure (or eligible to reciprocate) in the State of Delaware
