In the Population Health Clinical Pharmacist position, you will primarily function as a transitions of care pharmacist. You will provide transitional care management and pharmacy care management functions to members of ChristianaCare value-based care contracts (includes Medicare, Medicaid, and commercial lines of business).

PRINCIPAL DUTIES AND RESPONSIBILITIES:   

Patient Care:

  • Utilizes risk stratification tools to identify targeted high-risk patient population dependent on age, hospitalizations, ED visits, comorbid diagnoses (e.g. COPD, CHF, CKD), polypharmacy and behavioral health diagnosis.
  • Participates in CareVio meetings and forums.
  • Provides verbal and written consultations concerning drug therapy evaluations as requested.
  • Documents drug use and compliance patterns, detect and evaluate adverse drug effects, evaluate drug efficiency, therapeutic goals and indications, reviews for contraindications/precautions, the patient's current status of health, and review drug profiles and patient's medical record for drug-drug, drug-food, drug-laboratory and drug-disease state(s) potentials.
  • Evaluates appropriate laboratory tests necessary to monitor and support the patient's drug therapy program.
  • Documents assessment, observations, impressions, recommendations, plan, significant findings, interventions, and services rendered as appropriate in the care management and electronic medical record system.
  • Coordinates activities with health care team to perform enhanced medication reconciliation on high-risk populations and patients or as indicated by assessment.
  • Reports, records, assess and evaluate adverse drug reactions and communicate with providers to mitigate risk and/or develop a treatment plan.
  • Provides accurate, unbiased, timely and cost-effective drug information to patients of the health care team on targeted patient population.
  • Supports and participates in other duties as required to meet workload demands.
  • Serves as a resource to recommend pharmacotherapy alternatives or adjustments to providers to enhance compliance, reduce potential errors, adverse drug events and to minimize cost burden.
  • Appropriately convert/manage medications per P&T approved pharmacist policy, such as IV to PO conversion and renal dose adjustment, at the time of discharge.

Quality Improvement:

  • Works to integrate IT into workflow to enhance communication about medications across care settings.
  • Represent pharmacy on departmental and health system committees as assigned and contribute when pharmacotherapy issues are discussed.
  • Support and assist interdepartmental projects as requested (e.g., newsletter articles, performance improvement activities, etc.).
  • Participate in reporting ADEs to institutional committees and national programs (e.g., the FDA MedWatch program).
  • Participate in identifying and submitting events via Report2Learn (R2L) system and providing follow up assessment.

Research and Scholarship:

  • Participate in research design, data analysis, and publication of study results as a preceptor for resident research projects.
  • Contribute to the pharmacy and medical literature (e.g., case reports, letters to the editor, and therapeutic, pharmacokinetic, and pharmacoeconomic reports, peer reviewer).
  • Participate in regional, national, and international meetings for the pharmacy and medical community.
  • Contribute to pharmacy department and ACO newsletters as assigned.

Training and Education:

  • Coordinate educational lectures/meetings on medication therapy as needed.
  • Provide an inter-professional experience in training and mentoring pharmacy students and residents through experiential rotation as assigned.
  • Provide direct oversight for resident-related projects as assigned (i.e., grand rounds, MUEs).
  • Provide formal accredited and informal inter-professional educational sessions.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • PharmD from an ACPE accredited School of Pharmacy required
  • ASHP-accredited PGY-1 residency required
  • BLS Certification required
  • BPS board certification preferred
  • Current licensure (or eligible to reciprocate) in the State of Delaware
  • May require licensure in additional states to support ACO node partners
  • Travel to other locations to meet department needs
