As the Hematology/Oncology Clinical Pharmacist Specialist, you will provide pharmaceutical care to hematology and oncology inpatients at ChristianaCare by designing, recommending, monitoring and evaluating patient specific pharmacotherapy.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Round as a member of the interdisciplinary care team to provide comprehensive medication management for all patients
  • Review and interpret physician's medication orders, detect errors, drug interactions, drug incompatibilities, administrative drug use restrictions, or other indications of unsuitable drug therapy
  • Resolve such problems in consultation with the prescriber and nurse and clarify questioned orders prior to dispensing medication
  • Provide accurate, unbiased, timely, and cost-effective drug information to medical, pharmacy, nursing and other allied health professionals
  • Monitor drug therapy outcomes for targeted patients within established guidelines
  • Prospectively evaluate all drug therapy for appropriate indication, dose, drug interactions, drug allergies, and monitor the patient's pharmacotherapeutic regimen for effectiveness and adverse drug events (ADEs), and intervene as needed
  • Educate patients and/or patients' caregivers regarding medication therapies used to treat patients during and after acute illness
  • Provide routine stewardship activities targeted at anti-infectives and other medications, including those that may be high risk for adverse events, high-cost concerns, and inappropriate utilization
  • Document clinical activities that include, but are not limited to, disease state management, general pharmacotherapeutic monitoring, pharmacokinetic monitoring, ADEs, education and other patient care activities
  • Document in the medical record as appropriate as defined by departmental policy
  • Evaluate specific medication therapies consistent with departmental policy
  • Appropriately manage medications per P&T approved pharmacist policy
  • Verify medication orders in the PharmNet computer database for assigned unit during the first half of each daily shift
  • Participate in the investigation and resolution of missing dose issues on the unit when needed.
  • Demonstrate skill and knowledge necessary to provide pharmaceutical care for oncologic, hematologic and immunologic conditions
  • Manage patients supportive care plan with focus on pain, neuropathy, nausea and vomiting
  • Coordinate activities with health care team to perform enhanced medication reconciliation on select patient populations or as indicated by assessment
  • Collaborate with research team to support patients enrolled in clinical trials
  • Represent Department of Pharmacy Services on appropriate committees (Cancer Service Line and Stem Cell Transplant committee meetings)
  • Report, record, assess and evaluate adverse drug reactions
  • Identify and submit Report 2 Learn (R2L) events and providing follow up assessment
  • Provide individualized patient and family education regarding drug therapy
  • Support and assist interdepartmental projects as requested (e.g. MUE, P&T monographs, newsletter articles, performance improvement activities, etc.)
  • Develop and review evidence-based guidelines and order sets to ensure standardized, safe and effective treatments regimens
  • Participate in providing educational sessions (i.e., technician training, inservices, clinical pearls, staff development education) or materials for patient groups and allied health professionals
  • Assure handling of medications throughout the hospital complies with state and federal laws and regulations and hospital policies
  • Precept pharmacy residents and students, as assigned
  • Contribute articles to The Refill, the department's newsletter which also targets nursing staff
  • Prepare special medications involving compounding procedures and sterile products using aseptic technique when necessary to assure appropriate and timely dispensing
  • Provide pharmacist cross-coverage to maintain pharmacy services
  • Provide education on specialty medications prescribed by specialists
  • Address medication-related questions or concerns in-between provider visits as needed

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Pharm.D. degree required
  • ASHP-accredited PGY-1 Pharmacy Residency required
  • ASHP- accreditedPGY-2 Oncology Pharmacy Specialty Residency (or equivalent experience)
  • Board Certified Oncology Pharmacist (BCOP) preferred or obtained within 2 years of hire
  • Current licensure (or eligible to reciprocate) in the State of Delaware
