The Clinical Pharmacist is responsible for providing clinical pharmacy services for Oncologic, Hematologic, and Immunologic conditions primarily in the ambulatory setting. The pharmacist in this position provides extensive, direct pharmaceutical care in collaboration with providers in the Cancer Service Line, participates in drug therapy consults, clinical research protocols, and provides patient/family/staff/physician education, and precepts pharmacy residents and students.  To ensure comprehensive clinical pharmacy services in support of the continuum of care, regulatory standard compliance and patient care quality this pharmacist will have the following duties and responsibilities:


  • Design, implement and monitor patient-specific chemotherapeutic regimens, including supportive care in collaboration with the healthcare team (antiemetic therapy, acute/chronic pain management, nutrition support etc.)
  • Provide verbal and written consultations concerning drug therapy evaluations as requested
  • Provide education/counseling regarding drug therapy directly to patients and caregivers
  • Participate in discharge planning and facilitate continuity of care between the inpatient and outpatient settings, specifically for BMT and CAR T-cell patients
  • Lead the development of diagnosis specific order sets (electronic and/or paper) that reflect evidence-based recommendations and best practice
  • Provide oversight to Pharmacy Services' role with investigational drug studies at the HFGCC
  • Participate in daily activities of the HFGCC pharmacy
  • Report and evaluate adverse drug reactions
  • Participate in the educational experience of pharmacy practice residents and pharmacy students
  • Represent the department of Pharmacy Services on appropriate committees
  • Review and interpret provider's medication orders, detect and evaluate adverse drug effects, evaluate drug efficacy, therapeutic goals and indications, review for contraindications/precautions, the patient's current status of health, and review drug profiles and patient's medical record for drug-drug, drug-food, drug-laboratory and drug-disease state(s) potentials
  • Resolve such problems in consultation with the prescriber and nurse and clarify questioned orders prior to dispensing the first dose
  • Provide accurate, unbiased, timely, and cost-effective drug information to medical, pharmacy, nursing and other allied health professionals
  • Monitor drug therapy outcomes for targeted patients within established guidelines
  • Participate in the management and documentation of adverse drug reactions
  • Participate in identifying and submitting events via Report2Learn (R2L) system and providing follow-up assessment
  • Support and assist interdepartmental projects as requested (e.g. MUE, P&T monographs, newsletter articles, performance improvement activities, etc.)
  • Participate in providing educational sessions (i.e., technician training, in services, clinical pearls, staff development education) or materials for patient groups and allied health professionals
  • Verify non-formulary drug orders brought into the institution intended for use by provider conform to systemwide policy and labeling standards
  • Verify providers' medication orders in the PharmNet computer database and assure these orders are carried out
  • Participate in the investigation and resolution of missing dose issues on the unit when needed
  • Supervise activities of pharmacy technicians
  • Precept pharmacy residents and students, as assigned


  • PharmD required
  • ASHP-Accredited PGY-1 Pharmacy Practice Residency required
  • PGY-2 specialty residency in oncology/hematology required; or equivalent experience
  • Board Certification in oncology preferred
  • Current licensure (or eligible to reciprocate) in the State of Delaware
