Job Details / Primary Function
Christiana Care Hospital is currently looking to hire a Clinical Practice Coordinator (RN) to support Structural Heart procedural services for the Heart & Vascular Center.
The Clinical Practice RN Coordinator collaborates with multidisciplinary teams to provide clinical nursing expertise through all phases of care for patients with structural heart disease requiring interventional therapies. The RN coordinators serve as the primary patient and system resource and own patient tracking, pre-operative procedural coordination to include education and planning needs, data management, and process and program improvements and oversight. In addition, they facilitate required clinical follow ups, post-op updates, discharge planning and education in conjunction with the heart team. They serve as active participants in patient care coordination with the structural heart team, as well as hold responsibility for education to the health system, both internally and externally on structural heart program offerings and their value to the community and surrounding patient population.
The Structural Heart Disease Program at ChristianaCare was formed in 2011 as the first Structural Heart team in the state of Delaware to provide innovative and minimally invasive treatment options to complex cardiovascular patients. Our program features a multidisciplinary heart team approach, with physicians from different specialties bringing together their expertise to deliver patient outcomes that are consistently above the national average. Our structural heart team is comprised of interventional cardiologists, electrophysiologists, noninvasive cardiologists and cardiac surgeons, as well as our dedicated nurse coordinators, who are here to help guide patients through the evaluation process.
Work Schedule
EDUCATION AND EXPERIENCE REQUIREMENTS:
Graduated from an accredited nursing institution; Bachelor of Science in Nursing required.
Minimum of three years of cardiovascular nursing experience required.
SPECIAL REQUIREMENTS:
Active nursing licensure in the state of DE.
BLS certification required.
Certification in Nursing Specialty preferred.
