As the Clinical Staff Pharmacist, you will provide longitudinal pharmaceutical care to hematology and oncology patients at HFGCC infusion suite and physician practices by dispensing, overseeing preparation, monitoring, and evaluating of patient specific pharmacotherapy.

Principle duties and responsibilities:

  • Review and interpret provider's medication orders, evaluate drug efficacy, therapeutic goals and indications, review for contraindications/precautions, the patient's current status of health, and review drug profiles and patient's medical record for drug-drug, drug-food, drug-laboratory and drug-disease state(s) potentials
  • Coordinate and supervise work-place activities and resources; promote professional competence; assure that services rendered and work performed are in accordance with specific standards; advise, assist, and supervise work of Pharmacy Technicians; assume final responsibility for accuracy of their work; to include sterile product processing in the IV Admixture Service lab and ambulatory chemotherapy areas, where the pharmacist is responsible for assuring USP 797/800 standards are consistently in compliance
  • Resolve such problems in consultation with the prescriber and nurse and clarify questioned orders prior to dispensing medication
  • Provide accurate, unbiased, timely, and cost-effective drug information to medical, pharmacy, nursing and other allied health professionals
  • Monitor drug therapy outcomes for targeted patients within established guidelines
  • Demonstrate skill and knowledge necessary to provide pharmaceutical care for oncologic, hematologic and immunologic conditions
  • Manage patient's complete medication plan throughout the continuum of their treatment in concert with the health care team.
  • Manage patients supportive care plan (ie. nausea or vomiting)
  • Collaborate with research team to support patients enrolled in clinical trials
  • Report, record, assess and evaluate adverse drug reactions
  • Identify and submit Safety First Learning Reports (SFLR) R2L and providing follow up assessment
  • Prepare special medications involving compounding procedures and sterile products using aseptic technique

Education and experience requirements:

  • B.S. in Pharmacy or PharmD degree required; PharmD preferred
  • At least three years of acute care inpatient hospital pharmacy experience preferred
  • Outpatient infusion experience preferred
  • Sterile compounding experience preferred
  • Current licensure (or eligible to reciprocate) in the State of Delaware
