ChristianaCare's Community Benefit Team is seeking a Community Health Worker to promote and help navigate access to health and social services, provide health education, while advocating, facilitating, and organizing access to services with an identified community to improve the well-being of community members. This Community Health Worker will work with victims of violent trauma.

Principal duties and responsibilities:

  • Advocates for underserved individuals to receive appropriate services. Works to reduce cultural and socio-economic barriers between clients and institutions.
  • Attends training program provided by Christiana Care and the Penn Medicine Center for Community Health Workers.
  • Participates in staff meetings, local/national trainings and other meetings as requested.
  • Participates in community events promoting the Office of Health Equity and Community Health.
  • Performs assigned work safely, adhering to established departmental safety rules and practices; report to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
  • Discussing cases and case management in supervision with HVIP staff.
  • Works collaboratively and optimally with the interdisciplinary team to identify and address social determinants of health and community issues.
  • Making at least one face to face contact every week with participants in the community.
  • Builds connections between vulnerable populations, the health care system and community/state resources.
  • Establishes positive relationships with patients and their families, while assisting patients in the community and/or clinic setting.
  • Establishes and maintains professional relationships with community leaders, faith leaders and others responsible for providing services to the community.
  • Conduct intake interviews with patients/families, while identifying socio-economic issues that affect their overall health, and develop health/social management plans and goals.
  • Maintaining case file documentation as directed.
  • Collects data as required by supervisor.
  • Facilitates healthcare and social service system navigation.
  • Follow up with patients should be continuous from the initial identification through closure of the care management process.
  • Educates staff, health providers and partners about community needs.

Education and experience requirements:

High school diploma/ GED/ GED equivalent required. Bachelors preferred.

One-year community experience required; patient experience plus.

Public speaking, group facilitation and excellent writing skills preferred.

Computer experience required.

Demonstrated ability to work with diverse populations required.

Bi-lingual language skills are a plus.

An equivalent combination of education and experience may be substituted.
