ChristianaCare is currently seeking a part-time Community Educator for Kuumba Academy in Wilmington, DE.

    Primary Function:

    Community Health Workers promote access to services; provide health education and educate staff and community members in navigating the health care and social services systems.  

    Work Schedule

    • Monday-Friday, day shift (days will be discussed during interview)
    • This position is part-time, 24 hours/week for 42 weeks/year

    ** This position is grant funded; continuation of this position is contingent upon the grant being renewed.

    Principal Duties and Responsibilities:

    • Attends training provided by ChristianaCare.
    • Responsible for providing peer support as needed.
    • Develops and delivers health education to patients, students, and the community.
    • Creates connections between vulnerable populations, the health care system and community/state resources.  
    • Participates in team meetings with SBHC team and Community Health Workers. Assists patients in the school, community and/or clinic setting.  
    • Helps patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
    • Facilitates healthcare and social service system navigation.
    • Establishes and maintains professional relationships with community leaders, faith leaders and others responsible for providing services to the community. Participates in community events promoting The Medical Group and Community Health.
    • Educates staff, health providers and stakeholders about community needs. Facilitates educational events and/or small groups for patients, students, or community.  
    • Provides culturally appropriate health education on topics related to chronic disease prevention.
    • Advocates for underserved individuals to receive appropriate services.
    • Works to reduce cultural and socio-economic barriers between clients and institutions.
    • Collects data as required by manager
    • Works collaboratively and effectively within a team.  
    • Establishes positive, supportive relationships with participants and provides feedback to other members of the team.  
    • Builds and maintains positive working relationships with staff, clients, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds.  
    • Participates in regular program and department meetings.
    • Performs assigned work safely, adhering to established departmental safety rules and practices; report to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
    • Performs other related duties as assigned and required.

    Education and Experience Requirements:

    • High School diploma required;
    • Bachelor's Degree preferred
    • One year community and patient experience required.
    • One year of public speaking, and group facilitation.
    • Excellent writing skills preferred.  
    • Computer experience required. 
    • Demonstrates ability to work with diverse populations required.  
    • Bi-lingual language skills are a plus.  
    • An equivalent combination of education and experience may be substituted.

    **This position will require fingerprinting during the post-offer onboarding process.

    Special Requirements:

    • Access to personal vehicle. 
    • Ability to work flexible schedule, including some nights/weekends as needed. Knowledge of New Castle County.
