ChristianaCare is looking to hire a full time Community Health Educator to support Southern Delaware in Kent and Sussex Counties.

The successful candidate will be working full time in Kent and/or Sussex county on a daily basis Monday to Friday.

PRIMARY FUNCTION:

To provide education through Mid-Atlantic AIDS Education Training Center Regional Partner- Delaware to identify agencies in Kent and Sussex Counties that would benefit from education on HIV/AIDS, PrEP (HIV Pre-Exposure Prophylaxis), Hepatitis C, and various other topics.  This position will be an educator that focuses on training the community provider in an effort to improve clinical outcomes in people living with HIV. 

The MidAtlantic AETC recognizes the complexity of providing quality HIV/AIDS care, including the interplay among HIV clinical management, the care system, and the patient's needs. The MidAtlantic AETC responds to the intersecting issues of HIV, hepatitis C, sexually transmitted infections, substance use, and mental illness. The program focuses on improving prevention, care, and treatment of HIV and related disorders.

PRINCIPAL DUTIES AND RESPONSIBILITIES:   

  • Abide by all components of the scope of work and project work plan
  • Responsible for planning, implementation and evaluation of training programs, technical assistance and other activities related to the project
  • Coordination of the clinical content of training, development and implementation of curricula, consultation, and technical assistance
  • Develop materials and design program for conferences, workshops, and other educational activities 
  • Responsible for preparing reports, summaries and documentation related to training, consultation, providing technical assistance and outcome evaluations
  • Responsible for maintaining databases, data entry, providing reports and other data as needed
  •  Collection of relevant training, capacity building, and outcome data for reporting to Center Headquarters
  •  Prepares and submits applications for Continuing Education Credits to the Medical Society of Delaware (CME) and Department of Nursing Research and Education at Christiana Care (CNE) and tracks documentation for continuing education.
  • Also participate in relevant conference calls, committees, planning groups as needed both internally and externally.
  • This exempt employee may need to participate some in weekend and/or evening events over the course of the year.

SCOPE, PURPOSE, AND FREQUENCY OF CONTACTS:

Acts as a liaison between Mid-Atlantic AIDS Education Training Center and community agencies regarding services provided to the community agencies.   Frequent contact with healthcare support staff from various agencies to discern educational needs and create educational plan

EDUCATION AND EXPERIENCE REQUIREMENTS:

High School Diploma or equivalent required.  Bachelor's degree preferred.  Two years of community education/outreach experience preferred.

KNOWLEDGE, SKILL, AND ABILITY REQUIREMENTS:

Knowledge and skills to provide education to a variety of target audiences

Ability to work with minimal supervision

Ability to prioritize work assignments

Ability to work as a team player

Skill in interpersonal relationships

Knowledge of community resources

Skill in oral and written communication

Understanding of the needs of residents in Kent/Sussex counties

Ability to exercise judgment, tact and diplomacy.

Ability to be well organized and self-motivated.

Appropriately intervenes when policies or standards are violated.

Displays attitude of helpfulness, courtesy & cooperation toward participants, families, physicians & other personnel.

Ability to represent Christiana Care and the Holloway Community Program in a professional, positive manner at all times.

Verbal Spanish fluency a plus but not a prerequisite. 

SPECIAL REQUIREMENTS:

Ability to travel.  Strong understanding of how to meet the needs of underserved individuals.

PHYSICAL DEMANDS:

Frequent lifting, carrying, pushing, handling, reaching, twisting, walking, standing, and sitting.  Ability to travel.
