PRIMARY FUNCTION:

To provide leadership and management of operations of case management, social work, and utilization management functions and staff, including compliance with regulatory standards, conditions of participation, facilitation of care, transitions of care planning, utilization management and clinical revenue integrity, front-line quality and performance improvement initiatives.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Provides leadership in achieving organizational goals related to length of stay, excess/avoidable days, readmissions, and medical necessity/denials management.
  • Maintains positive relationships and collaborates with external stakeholders in order to maximize outcomes and regulatory compliance.
  • Promotes a professional practice environment that fosters teamwork and collaboration among team members. Ensures staff growth and development through mentoring programs and identifying and addressing gaps in competencies.

  • Ensures compliance with revenue/expense, capital equipment, renovation, and manpower operating budgets for assigned cost centers; monitors compliance with budget on a regular basis.
  • Performs assigned work safely, adhering to established departmental safety rules and practices; reports to the supervisor, in a timely fashion, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients or visitors.
  • Performs other related duties as directed.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Bachelor of Science degree in Nursing with professional licensure required.
  • Master's degree in nursing, Healthcare Administration, Business, Health Care Delivery Science or related field required.
  • Minimum 5 years of experience in Utilization Management, Case Management, Acute Care and/or Ambulatory Care required.
  • Minimum 3 years of supervisory/management experience
  • Certified Case Manager (CCM), Accredited Case Manager (ACM) or equivalent certification preferred.
  • Demonstrated capability in development execution of strategic initiatives.
  • Demonstrated track record of improving care outcomes.
  • Demonstrated capability in setting and delivering program goals.

SPECIAL REQUIREMENTS:

  • Active Delaware license.   CCM, ACM or equivalent certification required within 2 years.
