ChristianaCare's Project Engage /Addiction Medicine is looking for a Discharge Coordinator to join our team at the Wilmington Hospital.

How YOU Make an Impact

The Discharge Coordinator will contribute to the overall clinical operation of Project Engage and the daily care of patients with Substance Use Disorder (SUD). They will function as a member of the clinical team through collaboration, providing consultation, assessment, education and referrals to appropriate treatment.  They work closely with Project Engage Peer Counselors/Engagement Specialists and Addiction Medicine Consult liaisons on discharge planning for SUD patients. The Project Engage Discharge Coordinator will provide care coordination and assist high risk populations in overcoming social determinants of health that negatively impact their social, emotional, and physical well-being.

Duties and Responsibilities:

  • Work with the patient, family, staff, physicians, and other members of the health care team to provide care coordination.

  • Works with the patient, family and other members of the health care team to formulate a treatment plan that provides the patient services in the appropriate post-acute care setting.

  • Gathers and assesses information regarding the patient's physical needs, insurance coverage, mental status, family support system, financial resources, and available community and governmental resources. Employs assessment to develop a comprehensive care management plan that will address the needs identified.

  • Provides information about resources and options available in the community and coordinates service delivery. Interprets patient/family needs and provides information concerning the availability and limitations of resources. 

  • Identifies problems and gaps in community resources that impact outcomes and take leadership in effort to effect change.

  • Educates hospital staff on Project Engage services, the disease of addiction, SUD treatment resources in the community and barriers to treatment.

  • Maintains collaborative relationship with mental health/substance use disorder treatment providers to maintain current knowledge of community resources.

Education and Experience Requirements:

  • Masters degree in Social Work or Mental Health Counseling from an accredited graduate school required. A LMSW from the state of Delaware will be accepted in lieu of Masters degree in Social Work.

  • LCSW, LMSW, LPC or LMHC preferred.

  • Two years of experience working with mental health and/or Substance Use Disorder patients required. 

  • Experience doing discharge planning preferred.
