The Christiana Care Health System is currently recruiting for multiple Full Time Registered Nurse to join the newly created Hospital at Home team and work the day shift, 7am-7pm as well as every third weekend.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Perform and document accurate clinical assessments of patients on admission and throughout their hospital stay.

  • Identifies actual and potential problems for which the patient is at risk, demonstrating acquired knowledge/skill and a commitment to excellence.

  • Initiates, follows, and modifies a nursing care plan.

  • Individualizes teaching methods to best sit patient/family learning needs and abilities; revises educational plans to meet changing needs.

  • Initiates and participates in team meetings and interdisciplinary discussions regarding patient care or practice; elicits input from other team members.


EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Registered Nurse with BSN required; Graduate of an accredited school of nursing; BSN within 3 years of hire required

  • Active RN Licensure, in good standing and allowing RN work in Delaware

  • 6 months or more professional RN experience; med/surg experience preferred.
