ChristianaCare is currently seeking a LPN to work in our Discharge Lounge located at Christiana Hospital in Newark, DE. In this highly visible role, the selected individual will be responsible for assisting with the discharge of patients and partnering with our providers to ensure a safe handoff to their caregivers. This is an opportunity to be involved in the frontline care continuum to ensure a safe handoff to the community.

Responsibilities Include:

Facilitate patient movement at discharge to allow admissions and transfers to take place as we serve our acutely ill patient community, this involves physically bringing patients to a discharge lounge where the discharged patient will wait for their transportation home.

Utilizes technology to communicate to patient care units and monitor discharge flow throughout the hospital.

Primary responsibility will be to retrieve patients from discharging units and bring them to Discharge Lounge and provide minimal care for patients, if needed.

Reiterate discharge instructions, if needed.

Adapts communication style to persons representing diverse personal, professional, cultural and socio-economic backgrounds.


High school graduate or equivalent required. Graduate of an accredited school of practical nursing required.

Continuing of education to obtain RN and BSN degree through a scholarship agreement. (condition of employment).

Two years' of experience working as an LPN in an acute care/hospital or rehab facility required.

Hours: The hours for this position are 10 a.m.-6:30 p.m. or 11 a.m. - 7:30 p.m. and will vary at times based on volume.
