ChristianaCare, is looking for a Sr Social Worker I for our School Based Health Center located at William Penn High School  

School-Based Health Centers were established especially for students in order to help make the best quality health care available in our community more accessible to them. Because they are located right in school, School-Based Health Centers help students overcome many obstacles to receiving good health care - obstacles such as lack of transportation, inconvenient appointment times, or worries about cost and confidentially. School-Based Health Centers provide comprehensive medical and mental health care, treatment and health education to promote a healthy lifestyle. Each health center is operated by a partnership that includes ChristianaCare Health System, school districts and the Delaware Division of Public Health, Department of Health and Social Services.


To contribute to the overall clinical operation of the site-specific School Based Health Center (SBHC). and the daily care of the youth and young adults' populations of the various schools and the overall functioning of the SBHCs and Family and Community Medicine Department.  To function as a member of the SBHC integrated clinical team through collaboration, providing consultation, assessment, treatment and behavioral health education.  To provide collaboration with and a conduit to Behavioral Health and support an integrated model of health care


  • Provides psychological care (consisting of individual, group and family therapy interventions as well as Crisis Interventions) appropriate for the youth and young adult population.  This requires the ability to conduct assessments, obtain and interpret information, to identify age and culturally-specific patient needs, and to provide the care needed.
  • Functions in a consultation and liaison role for the Family and Community Medicine Department, the SBHCs and the individual Schools, meeting the needs of the overall Health System, the SBHCs and the students.   
  • Utilizes and scores various psychotherapeutic screeners using appropriate  evidenced-based clinical techniques (RAAPS, Columbia Suicide Screen, BDI-II, BAI, PHQ-9, etc, and CBT, DBT and Crisis Centered Therapy to name a few) to meet the needs of the youth and young adult population. 
  • Coordinates transition of care to community setting as necessary; maintains knowledge of resources including community providers and support organizations appropriate for the patient population served
  • Provides quality clinical care and maintains documentation pursuant to the requirements of insurance and billing requirements using ICD0-10 coding with appropriate clinically justified diagnostic criteria
  • Collaborates with School and consults with Special Education Providers with 504/IEP planning as necessary for students who have Special Education considerations.
  • Develops group support and therapy activity (Individual, Group and Family) as appropriate for the specific Medical or Specialty population, and as needed within the Christiana Care Health System.
  • Develops a clinically relevant treatment plan in conjunction with the student to address the identified emotional needs.
  • Contributes to the development of staff (SBHC and School) by providing consultation, education and suggesting best practice alternatives as appropriate.
  • Maintains connection and consultation with the Department of Psychiatry and Behavioral Health
  • Maintains established productivity levels (See at least 6 students per day when school is in session)..
  • Creates an environment that encourages and invites family involvement while respecting the student's confidentiality rights.
  • Identifies service gaps pertaining to patients' psychosocial needs and plans programs that address the psychosocial needs of patients.
  • Complies with all goal and expectations and deliverables as outlined by the DE Department of Public Health Contract, including meeting and following all State SBHC Regulations.
  • Implements programs effectively and evaluates effectiveness of programs, making modifications as needed.
  • Assesses learning needs of patients and families and carries out treatment as indicated.
  • Provides chaperone coverage for medical provider as outlined in the CCHS Chaperone Policy-Outpatient.
  • Develops and provides patient education as appropriate.
  • Provides system-wide presentations as applicable
  • Participates in all Performance Improvement Initiatives within the SBHC as appropriate and attends/presents at National Professional Organizational Meetings as needed


  • Knowledge of behavioral health principles including assessment, treatment, diagnosis, consultation and terminology
  • Knowledge and ability to conduct brief consultative and formal therapies
  • Ability to utilize effective listening skills and remain empathetic and developing strong working alliances
  • Ability to procure, clarify, and interpret information on verbal and non-verbal levels
  • Knowledge and ability to facilitate a positive, caring, and supportive atmosphere for positive change in all patients.
  • Skill in written and oral communication
  • Knowledge of patients' rights, ethics, and legal issues
  • Strong working knowledge of the Diagnostic and Statistical Manual (DSM) and ICD Codes
  • Knowledge, Understanding and Acceptance of the Social Work Code of Ethics
  • Knowledge of Crisis Intervention Theory


  • Education: Masters in Social Work , MS/MA in Counseling or equivalent masters degree.
  • Licensure: Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor Mental Health (LPCMH), Licensed Marriage and Family Therapist (LMFT),  
  • One to five years of experience providing direct therapeutic care
