Are you a NP/PA that has a passion to work with patients with special care needs?  ChristianaCare is currently looking for a Primary Care NP/PA that wants to join our team of caregivers to give the best care possible to the special needs population.  This is a Monday-Friday position with on-call requirements and is located at our Wilmington Campus.   Don't miss out on this amazing chance to join us!

            

             PRINCIPAL DUTIES AND RESPONSIBILITIES: 

  • Provide patient care with a focus on sickle cell and intellectual disability
  • Collaborate and coordinate patient infusion needs with the CCHS Infusion center at Wilmington Hospital and Christiana Hospital
  • Provide infusion center and urgent visits
  • Collaborate and coordinate annual wellness visits and post hospital discharge follow-up visits
  • Assess and document patient and family response to intervention
  • Demonstrate ability to carry out critical skill as outlined in procedures and specific standards
  • Prescribe medications in a safe and efficient manner
  • Establish priorities based on patient needs and emergent care
  • Independently review patient problems including, but not limited to medication refills, pain management, determining the need of follow up care and referring appropriately to the physician according to policy
  • Perform all prescribed laboratory test, send to appropriate laboratory, receive and interpret laboratory values, and notify physician of results
  • Provide patient education and counseling to patients and their families
  • Manage patient's chronic disease inpatient with collaboration of hematologist

     
    EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Master's degree in Nursing with Family Nurse Practitioner Board Certification or graduate of an Accredited Physician Assistant program.
  •  1 year of experience as Nurse Practitioner or a Physician Assistant preferred
  • Licensed or eligible for license in the State of Delaware
  • BLS Certification
