Are you a Nurse Practitioner or Physician Assistant that is ready to take on an innovative role and be part of a great team?  ChristianaCare is looking for a  full time NP or PA to work in our Center of Virtual Health.  The selected candidate will provide virtual continuity and comprehensive primary care to a panel of patients across the lifespan. The NP/PA will able to grow their own panel of patients and work closely with a multidisciplinary team.  This practice is located at our new Avenue North location in Wilmington, DE with the potential when clinically needed to provide home based care.  The practice offers expanded hours and 7 day a week clinical access with practice hours between 7am-8pm Monday-Friday and 9a-1p on Saturdays.  The selected candidate will rotate extended hours on weekdays and weekends, as well as on-call between the other providers.

        

DUTIES AND RESPONSIBILITIES

  • Assesses health status of patients through virtual physical examination and medical history.
  • Analyze patient data and formulate an individualize plan of care based on patient assessment and needs.
  • Evaluate and monitor the effectiveness of prescribed management plan.
  • Independently review patient problems including, but not limited to medication refills, determining the need of follow up care and referring appropriately to the physician according to policy.
  • Provides medical management of patients in the patient's home, including facility based homes. This includes seeing patients in a professional manner, obtaining accurate history, performing a thorough physical exam, forming a diagnosis, developing, and executing appropriate treatment plans, and ensuring adequate follow up care.
  • Perform necessary diagnostic and therapeutic procedure at patient's home or center's access points.
  • Administer medical care in a safe and efficient manner consistent with regulatory and organizational policies and procedures.
  • Establish priorities based on patient needs and act on these accordingly.
  • Transcribe orders appropriately and within scope of practice and credentials.
  • Order necessary diagnostic and therapeutic procedures within scope of practice and privileges.
  • Complete appropriate and timely documentation in the electronic health record (EHR), which includes history  and physical (H&P), ambulatory office notes, and message center.
  • Submit billing in a timely fashion according policy.
  • Utilize teaching/learning principles coupled with an expert clinical knowledge to promote and provide education and counseling to the patient and/or family including teach back.
  • Participate in performance improvement, research, and/or organizational activities.

EDUCATION AND EXPERIENCE REQUIREMENTS

  •  Completion of a Master's degree program and board certification as a Family Nurse Practitioner or completion of a Physician Assistant program and board certification as a physician assistant. 
  •    At least 2 years of APC experience preferred
  •  Experience with delivering care in a Virtual platform preferred
  •   BLS Certification
  •    License or license eligible in the states of Delaware, Maryland, New Jersey and Pennsylvania
