ChristianaCare is currently looking for a NP or a PA to work with our Memory Care team at our new Rehoboth Beach/Milford practices. We are seeking a suitable candidate with experience and enthusiasm for treating adults with dementia and providing resources to their primary care clinicians and  home caregivers. The selected candidate in turn is supported by supervision from the Swank Center for Memory Care and Geriatrics and is also able to access peer support and guidance from the DCS community of the ADC's multiple sites.

The ADC (Alzheimer's and Dementia Care) program embeds a specially trained NP/PA in large primary care practice settings for the purpose of supporting the detection, diagnosis, treatment and primary care management of adults with clinically meaningful cognitive impairment. The success and value of this program in improving care of our patients and support of their caregivers has been so notable that we are expanding the ADC to Lewes and Milford.

Core Duties and Responsibilities

  • Assesses health status of patients through complete physical examination and medical history.
  • Analyze patient data and formulate an individualized care plan based on patient assessment and needs.
  • Initiate or adjust patient medications in collaboration with the PCP.
  • Make referrals for additional community resources as needed, matching the patient and their caregivers based on need, cost, and geography, while reducing the burden on the PCP, such as social work, physical and occupational therapy, home nursing services, hospice, and other specialists as needed including Swank Center.
  • Help coordinate transitions of care to maximize quality of life and functional status for the patient and family. 
  • Contribute to program development and utilization of best practice guidelines, quality and performance improvement protocols, and research.

Education and Experience Requirements

  • Graduate from an accredited APRN program and certified as either FNP or AGNP
  • Physician Assistant must be a graduate of an ARC-PA accredited physician assistant program and have NCCPA Certification
  • Prescriptive Authority
  • 2-3 years of experience with adults, preferably geriatric patients
  • Experience with Memory loss patients preferred
