ChristianaCare is searching for an experienced Nurse Practitioner (FNP or AGNP) or Physician Assistant to join the Swank Memory Care Center, located in the Gateway Building of the newly renovated Wilmington Hospital. 

The Swank Memory Care Center is the first and only comprehensive outpatient program in Delaware for patients with memory disorders and their families. The Swank Memory Care Center, built on a model of patient- and family-centered care, is collaboration among the departments of Medicine, Family & Community Medicine, and Psychiatry & Behavioral Health.

This is a full-time position offering a flexible schedule working 4 10hr shifts per week.  The selected candidate will perform outpatient consults as well as assisting primary care providers with the management of memory care patients in their practice through a consultative design approach. The Swank Memory Care team is multidisciplinary bringing together memory specialists, neurologists, psychiatrists, and social workers to provide care for patients and families.

Education and Experience Requirements

  • Graduate from an accredited APRN program and certified as either FNP or AGNP
  • Physician Assistant must be a graduate of an ARC-PA accredited physician assistant program and have NCCPA Certification
  • Prescriptive Authority
  • 2-3 years of experience with adults, preferably geriatric patients
  • Experience with Memory loss patients preferred
