PRIMARY FUNCTION:

The registered nurse utilizes advanced clinical skills to coordinate and direct staff to insure the delivery of high-quality patient care services for the Primary Care patient population in an office practice setting. The nurse will provide high quality, safe, evidenced-based nursing care to achieve optimum outcomes both independently and as a valued team member.  The nurse will follow the Nursing Process when providing patient care.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Administrative Duties

  • Assists Practice leadership with interviews, selects, and orients qualified clinical candidates
  • Collaborates with the NPDS to ensure system/practice required new hire orientation, onboarding, and trainings are completed
  • Orients new clinical staff to clinical procedures
  • Assures compliance with organizational, quality and safety regulatory requirements
  • Assists Medical leadership in developing and implementing performance improvement initiatives
  • Attend all site management meetings and trainings and represent the practice in Medical Group Initiatives
  • Collaborates with the site supervisor with completing quarterly and annual caregiver performance reviews
  • Participates in following up on patient/family/physician concerns and effectively address and intervenes, as indicated, to address and resolve difficult, stressful, or volatile patient situations
  • Follow up on R2L Alert System to ensure issues are addressed and closed in the system
  • Performs chart audits or has designee assist with chart audits
  • Audit and monitor checkpoint refrigerator & freezer monitoring system
  • Audit QA and POCT Testing logs
  • Randomly shadows clinical support staff to ensure appropriate care is provided
  • Mentors and acts as a resource for other registered nurses and team members to encourage development of their interdisciplinary approach and delegation skills 
  • Collaborates with team members to provide an organized and efficient office workflow
  • Prepare weekly staff schedule and ensure all providers are supported (daily) and make staffing adjustments due to call-outs, emergencies.
  • Work with Nurse Managers and NPDS on MA education and development
  • Provide oversight and leadership for clinical projects and initiatives (Flu clinic, LARC, Huddles, ED follow up, Population Health, etc) in tandem with Nurse Manager and Practice Manager
  • Maintain regular touch points (weekly and daily) with Senior MAs to support ongoing staff development and communication
  • Facilitate weekly team huddles/monthly meeting and provide clinically related updates
  • Provide practice wide clinical updates and notifications to all staff/providers
  • Escalate operational concerns to practice supervisor and practice manager
  • Perform other related duties as required

Clinical Duties

  • Supervises and performs clinical nursing duties as needed; including but not limited to taking vital signs, administration of medication, point of care testing, triage,  EKG, patient education, appointment scheduling, wound care documenting patient interactions in patient's medical record and other clinical tasks as directed by clinicians
  • Supervises and performs Transitional Care Management Interactive calls as needed which includes medication reconciliation, plans for follow-up visit and provides or arranges for patient education regarding specific health care skills and general disease concepts
  • Supervises and performs Chronic Disease Management and assist with performing billable Annual Wellness utilizing unique knowledge, skills, and competencies required to promote wellness, restore health, and support end of life care.  
  • Identifies infection prevention and patient safety concerns for the practice
  • Escalates nurse practice concerns to the Manager of Nursing Services
  • Supervises triaging of desktop messages and phone as needed
  • Performs assigned work safely, adhering to established departmental safety rules and practices
  • Reports to Practice Manager and Manager of Nursing services, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors
  • Performs other related duties as required

Daily contact with patients, family members, physicians, medical assistants, Office assistant, volunteers, students, community agencies, other hospitals and the general public

Provides direction to RN, LPN, and MA

DIRECTION/SUPERVISION RECEIVED:

Manager of Operations, Manager of Nursing Services, Clinical Medical leaders

EDUCATION AND EXPERIENCE REQUIREMENTS:

Bachelor's degree in Nursing required

3 years professional nursing experience

2 years' experience as a RN, physician office experience preferred

Clinical supervisory experience preferred
