Patient Digital Ambassador, Hospital at Home

The Patient Digital Ambassador (PDA) for the Hospital at Home program is an integral member of the Hospital at Home Command center, ensuring operations run smoothly by provider virtual administrative and logistical support to the multidisciplinary clinical team. The PDA for Hospital at Home is responsible for coordinating and scheduling home visits, equipment delivery, and tab tests with community partners for Hospital at Home patients, communicate and follow up on key administrative tasks for the command center team.

Hours of Operation 7:00-8:00pm, with a weekend rotation

There will be 2 shifts: 7:30am -4:00pm and 11:30am to 8:00pm

Principal Duties and Responsibilities

  • Collects and organize patient data
  • Maintain records and databases including records and databases including record set up and maintenance
  • Upload paperwork to patient record including instance information
  • Coordinate and schedule home visits, equipment delivery, and lab tests with community partners and vendor from order to service fulfillment
  • Diligently follow -up on administrative tasks and communicate updates to the team in a efficient and reliable manner
  • Manage complex and multidisciplinary real-time scheduling needs of a diverse patient population
  • Maintain supplies and documentation, as needed, to support the unique Hospital at Home Delivery model
  • Collaborate with leadership to develop and implement new processes and systems, with patient to train and orient new staff
  • Perform other administrative support duties as required

Education and Experience Requirements:

  • Associate degree in health related field required, Bachelor degree preferred
  • 2 years' experience in healthcare related field or Medical Assistant field preferred
