PRINCIPAL DUTIES AND RESPONSIBILITIES:
Collaborates with psychiatrists, psychologists, physicians, therapists, community-based services and supportive personnel regarding treatment planning, referral and discharge planning to facilitate transition throughout the continuum of care and progress in treatment.
Assesses needs, supports, barriers, and available community resources to address problems related to social determinates of health.
Generates and maintains professional contacts with recovery-oriented treatment programs, support groups, medical services and community agencies in order to facilitate appropriate referrals.
Obtain releases of information, prepare documents, complete forms and send or receive confidential medical information required to facilitate referrals and coordination of care.
Participates in departmental meetings, multidisciplinary team meetings, and contributes to performance indicators for the program.
Provides coordination of care activities with patients, families, providers, payors and other involved parties.
Completes prior authorizations and utilization management for services.
Provides educational information to patients and family members including assistance with accessing technology that enhances participation in services.
Distributes naloxone kits and provides education on their use to patients and families. Maintain distribution records, as required.
Monitors Open Beds, program email, and internal messages, responding to requests for care coordination in a timely manner and completing appropriate documentation for all patient related contacts.
Schedule/reschedule appointments for providers, therapists and group therapy.
Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
SCOPE, PURPOSE, AND FREQUENCY OF CONTACTS:
Consistent daily contact with patients, families, medical and other professional staff from various Christiana Care departments and community agency representatives.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Bachelor's Degree in Social Work, Behavioral Sciences or related field.
At least two years' experience working with patients diagnosed with mental health and/or substance use disorders.
An equivalent combination of education and experience may be substituted.
KNOWLEDGE, SKILL, AND ABILITY REQUIREMENTS:
Knowledge of Social Work, Crisis Intervention, and Human Behavior theories and practices.
Knowledge of Community Resources and processes for accessing services.
Ability to function as a member of a multidisciplinary team.
Ability to organize and prioritize work assignments.
Ability to maintain flexibility and professionalism while working with complex patients who may demonstrate physical or behavioral impairment.
Maintain timely and appropriate documentation.
