We are currently seeking a Full Time Referral Service Manager.  This person provides accountability and responsibility for excellence in the operational office setting and clinical practice of care transitions in an ANCC Magnet® designated Christiana Care Health Services area. The Referral Service Manager is responsible for managing patient referrals from hospitals and other referral sources to ensure a smooth transition of care for patients being admitted to Christiana Care HomeHealth.  The successful candidate will work the hours of 8am - 4:30pm.  Will require an occasional weekend and holiday shifts, as well as some on call shifts.

PRINCIPAL DUTIES AND RESPONSIBILITIES:                                               

  • Demonstrate knowledge and skills necessary to provide care to adolescent, adult and geriatric patients

  • Develops and maintains methods and processes to ensure exceptional patient satisfaction and customer experience

  • Routine supervision of approximately 20 HomeHealth clinical and non-licensed professionals

  • Providing clinical record documentation review, case conference and patient rounding to establish visibility, teamwork and monitoring performance

  • Supports and communicates system-wide and agency based Performance Improvement initiatives

  • Promotes the autonomy of the clinical staff, encouraging independent decision making and problem solving

  • Responsible for service recovery and analyzing gaps in quality of care transitions


  • Bachelor's Degree in Nursing required; Master's Degree preferred and must agree to complete within 3 years of start

  • RN licensure in State of DE, or an appropriate compact license

  • BLS

  • 3 years of management experience or demonstrated progressive leadership development

  • 2-3 years of Home Health/Hospice, or Case Management experience
