As a Full Time 40 hours a week Nurse Liaison for ChristianaCare HomeHealth you will identify home care needs of patients to ensure a smooth transition from acute care and post acute care settings as they transition to their home environment. The Nurse Liaison supports the intake process by interacting with discharge planners, case managers and patients/families within the acute and post acute care settings.
The successful candidate will engage with clinical personnel who make referrals for home health care. The Nurse Liaison will be based in our Wilmington Hospital Campus, work 5 days a week Monday through Friday 8am-4:30pm, working with other liaisons and members of the interdisciplinary team. This person will be expected to acquire a minimum of 40 referrals a week and will also be required to assist with weekend coverage, 1 out of 8 weekend days, and 1 holiday each year. Our Part Time position requires the successful candidate to work 32 hours a week, 2-3 days a week on site at the Wilmington Hospital Campus, as well as 2-3 weekends a month. Our part time position is eligible for benefits.
Requirements:
- RN must either possess a BSN or be willing to acquire within three years of start date
- Previous experience with HomeHealth is preferred
- Strong interpersonal and customer service skills
- Excellent communication and organization skills
- Ability to work and travel to hospital and post acute care settings
