ChristianaCare, Medicare Advantage (Primary Care for patients age 65+) at Milford is looking for a part-time (0.5) Registered Nurse. This position has the potential to grow into a full-time position.

The Medicare Advantage (65+) practice at Milford offers a new innovative model of healthcare, specific to the geriatric patient population. The 65+ practice(s) will provide comprehensive medical care to Highmark Medicare Advantage patients. This unique model of care focuses on patient-centered, individualized care plans and standardized clinical outcomes.

This practice will be the first in Delaware to offer unique services designed specifically for the geriatric populations with features such as a Functional Assessment Room, therapy for ADL support, on-site access to a pharmacist, social worker, nutritionist, as well as a private family gathering room. In addition to services being offered at the practice, we'll have a home visit component for our home bound patients in the surrounding area. By leveraging technology and the on-site support of our digital ambassador, we will provide a large virtual component for primary and specialty services. In addition to traditional Primary Care services, the practice will offer functional assessments performed by a Physical Therapist, on-site behavioral health and nutrition services and much more!

We are a team of one Physician, one Advanced Practice Clinician, supported by a Registered Nurse, Pharmacist, Social Worker, Medical Assistants, Office Assistants and a Patient Digital Ambassador. The Registered Nurse will function as a member of this clinical team through collaboration with other healthcare team members to contribute to the overall clinical operation of the department and the daily outpatient and home bound patient population through the nursing process.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Implements care in a knowledgeable, skillful, consistent, and continuous manner.
  • Functions to establish priorities of patient care based on essential patient needs and available unit resources of time, personnel, equipment, and supplies.
  • Functions as a liaison between patients, providers, and staff to provide accurate communication
  • Assists provider with examinations and procedures.
  • Performs various point-of-care / laboratory tests; reads and records results.
  • Accurate medication reconciliation and help patients manage their medication safely and effectively.
  • Document in CCHS electronic medical record (EMR) system in a timely and accurate manner.
  • Functions with an awareness and application of safety policies and in compliance with Infection Control policies.
  • Performs effectively in emergency situations following established protocols, remaining calm, informing appropriate persons, and documenting events.
  • Demonstrates sound knowledge base and actions in the care and decision making for designated patient population.
  • Demonstrates personal responsibility and accountability for own professional practice at all times.
  • Demonstrates awareness of legal issues and patient rights in all aspects of patient care and strives to manage situations in a reduced risk manner.
  • Actively participates in the development of department, and Nursing goals.  Maintains required credentials of CPR, mandatory inservice education.
  • Demonstrates effective communication methods and skills, using lines of authority appropriately.
  • Formulates and maintains effective working relationships with all health care team members.
  • Maintains an attitude of helpfulness, courtesy, and cooperation towards patients, families, physicians, and other departmental/hospital personnel.
  • Demonstrates skills and knowledge necessary to provide care to adult patients, including the ability to obtain and interpret information to identify patient needs, and to provide the care needed.                             
  • Performs assigned work safely, adhering to established departmental safety rules and practices. 
  • Reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
  • Assists support staff with administrative tasks, as needed
  • Performs other related duties as required.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • BSN preferred; ADN considered with agreement of completion of BSN in 3 years
  • A graduate of a state approved or National League of Nursing accredited School of Nursing
  • RN license or eligible for licensure in the state Delaware
  • Two years' experience as a RN, physician office and/or geriatric experience preferred
