As a Sr Social Worker you will work to enhance and improve the quality of patient care at Christiana Care by providing assessment, treatment, consultation and educational services for individuals with perinatal mood disorders. Provides clinical support to the Center for Women's Emotional Wellness (CWEW) practice within the Behavioral Health service line.

Senior Social Worker would be responsible for :

  • Provides coordination of care and treatment services for Center for Women's Emotional Wellness, in collaboration with Behavioral Health and Women's Health.
  • Collaborates with multidisciplinary team (psychiatrists, psychologists, physicians, nurses, case management) regarding treatment planning and connection to community-based services as appropriate.
  • Conducts clinical interview to provide diagnostic impression, treatment recommendations to perinatal patient population.
  • Provides evidenced-based psychological inventions for perinatal mood disorders.
  • Provides coordination of care services with patients, families, providers, payment sources and other involved parties.
  • Maintains up to date information on evidenced based strategies and interventions to address psychological issues related to the perinatal period.
  • Participates in departmental and hospital wide committees and contributes to AOP goals, Ways of Working and other process improvement projects.
  • Provide community education regarding perinatal mood anxiety disorders and other health care system initiatives (e.g. trauma informed care)
  • Links patient/ family with available community and government resources to meet their identified needs. Assists the patient and family in all aspects of accessing resources including but not limited to advocating with agencies on their behalf, working with other facilities on support planning.
  • Available for assessment of mental health issues, consultation and referral assistance throughout Behavioral Health and the system at large as assigned.
  • Able to effectively communicate and work constructively with the multidisciplinary treatment team and maintain timely and appropriate documentation of the patient's clinical status and progress and treatment.
  • Able to deal effectively with both medically and psychiatrically ill patients who may exhibit physical and/or behavioral dysfunction.
  • Performs assigned tasks that need to be completed even when outside the scope of routine responsibilities; adaptable to changing departmental conditions.

Work Hours

Monday to Friday - 8am to 5pm

License and Education Requirement

  • Licensed Clinical Social Worker (LCSW) OR LMSW required
  • 3+ years of current, or recent experience in a health care setting; Community work preferred
  • Direct experience in providing evidenced based assessment and treatment for perinatal mood disorders
  • Perinatal Mental Health Certified- preferred.
  • An equivalent combination of education and experience may be substituted.
