Christiana Care is looking for a full time Senior Social Worker for the Holloway Program.

As a social worker you would facilitate and coordinate the behavioral health care of patients (adolescent, adult & geriatric) infected with HIV. You works to assist patients and their families to adjust to personal and environmental difficulties which predispose illness or interfere with medical care.

In addition, you would provides mental health assessments and treatment  under direction of the program psychiatrist and in collaboration with the medical supervisor in keeping with defined treatment goals.


To facilitate and coordinate the behavioral health care of patients (adolescent, adult & geriatric) infected with HIV. Works to assist patients and their families to adjust to personal and environmental difficulties which predispose illness or interfere with medical care. Provides mental health assessment and treatment  under direction of the program psychiatrist and in collaboration with the medical supervisor in keeping with defined treatment goals.


  • Maintains a current knowledge base of HIV disease process. Utilizes knowledge to appropriately assess patient behavioral health needs and implements or facilitates appropriate intervention. Works appropriately with health care team to coordinate care on an outpatient basis when necessary.
  • Performs, documents and bills for Psychiatric Evaluations W/O Medical Services for patients referred by the Holloway Program treatment team. Establishes DSM-5 and ICDl0 diagnosis based on evaluation and establishes a treatment plan. Provides on-site counseling based on established clinical methods.
  • Maintains pertinent and timely documentation, including assessments and progress notes according to CCHS standards for billing of services. Accurately maintains statistical data. Collects and maintains information for performance improvement initiatives as required.
  • Responsible for coordination of behavioral health services in collaboration with Holloway Program psychiatrist. Develops linkages with community mental health providers and facilitates referrals and communication with the Holloway Program staff. Evaluates efficacy of treatment plan to insure optimal treatment benefit.
  • Provides Behavioral Health Counseling to patient's with HIV and other chronic medical conditions (OM, HTN, Obesity) to improve health outcomes and maintain adherence behaviors.
  • Provide crisis intervention counseling for patient's experiencing a psychosocial, psychiatric or health crisis.
  • Provide "in room" consultation at the request of other providers or team members to establish a relationship, stabilize a crisis, or provide short-term behavioral health interventions.
  • Provide consultation to providers and team members regarding mental health treatment and treatment planning including psychotropic medication recommendations for various presenting problems when requested.
  • Provide Substance Abuse assessment, treatment and referrals based on principals of motivational interviewing. Consult and collaborate with the Program Psychiatrist on the treatment of shared patients.
  • Maintains current knowledge of resources available to assist other health care providers and Holloway program staff in coordination of care.
  • Collaborates with case managers and community agencies to provide a comprehensive continuum of patient care and to maximize utilization of available resources. Provides education and training as requested
  • Functions as a representative of the Holloway Community Program and Christiana Care Health Services, interfacing with appropriate community and professional agencies.
  • Acts as a resource person and assists in educational efforts involving HIV, with particular  emphasis on psychosocial , substance abuse and mental health aspects. Precepts students at various levels as appropriate and serves as a field instructor when able.
  • Adheres to Christiana Care Health Services policy with regard to release of information, patient confidentiality and requests made by the news media. Discerns when confidential information should be shared in order to facilitate a multidisciplinary health care practice.
  • Demonstrates awareness of self-learning needs. Coordinates goals with Program Director. Interfaces with resource people in order to provide expert client care and facilitate self-development. Maintains continuing education ad clinical expertise for licensure.
  • Represents Christiana Care Health Services and the Holloway Community Program in a positive and professional manner.
  • Demonstrates skills and knowledge necessary to provide clinical mental health care appropriate to adolescent. adult & geriatric patients, including knowledge of growth and development, the ability to obtain and interpret information to identify patient needs, and to provide the care needed.
  • Performs assigned work safely, adhering to established departmental safety rules and practices. Reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
  • Attends mandatory education programs annually including Fire/Safety Program, Infection Control Program, Right to Know Law Program, and CPR Certification.
  • Performs other related duties as required.


  • LCSW Preceptor (if LMSW)


  • Master's degree in social work from an accredited graduate school of Social Work or comparable education
  • LMSW may be considered with requirements to obtain LCSW within 2-years or LPCMH for specific populations
  • Demonstrated interest in HIV patient management


  • Knowledge of Clinical Social Work Theory and understanding and acceptance of Social Work Code of Ethics
  • Knowledge of HIV disease process including epidemiology, treatment modalities, and research protocols Knowledge of Crises Intervention Theory
  • Knowledge of Human Behavior and Developmental/Life Cycle Theory
  • Knowledge of community resources, state and federal programs Skill in oral and written communication
  • Skill in interpersonal relations
  • Ability to demonstrate Cultural Competence
  • Ability to communicate effectively with physicians, staff, patients, families as well as community and volunteer agencies
  • Ability to act as a role model in the maintenance and protection of patient confidentiality
  • Ability to assess when information should be shared with health care providers in an effort to facilitate patient care
  • Ability to function as a member of an interdisciplinary team
  • Ability to learn and understand medical terminology, Psychiatric and Psychological terminology. Ability to organize and prioritize work assignments, which may be extensive at times


  • Licensed by the state of Delaware as an LCSW or Licensed Professional Clinical Mental Health Counselor (LPCMH)
  • LMSW that want to obtain LCSW hours will need to adhere to the guidelines to obtain licensing hours
