We are currently seeking a Full Time Senior Social Worker to join our HomeHealth team.  The Social Worker is responsible to the Care Management supervisor and works in close cooperation with nursing staff and Care Management Supervisor for the implementations and deliver of Medical Social Work services.  The successful candidate will work the hours of 8am - 4:30pm.  Will require an occasional weekend and holiday shifts.

Principal duties and responsibilities:

  • Assist the physician and other members of the health care team in understanding the significant social and emotional facts related to the patient's health.
  • Make home visits to assess the social and emotional factors in order to extimate the patient's capacity and potential to cope with challenges.
  • Help the patient and patient's family to understand, accept and follow medical recommendations and provide services planned to restore the client to optimum social and health adjustments within his/her circumstances.
  • Assist patient and families with personal and environmental difficulties which predispose toward illness or interfere with obtaining maximum benefits from medical care.

The successful candidate will possess a Master's Degree from a school of social work accredited by the council on Social Work Education, and one year experience in health care setting.
