ChristianaCare's Medical Group is looking to hire a full time Senior Social Worker I with the Social Care Connection Center .
The Senior Social Worker is a member of the Social Care Connection Center who work to identify and patient social, emotional, and environmental needs and connect them to available resources and services to improve patient care. The Senior Social Worker I will be based out of Avenue North, Wilmington.
Work Schedule:
Monday- Friday: 8am- 4:30pm.
This position is a hybrid blend of remote/virtual and in person work.
No weekends/late nights.
Key Responsibilities:
Participates as a member of the Social Care Coordination Center, focused on the addressing needs and barriers of patients with social care complexity.
Collaborates with Community Health Workers on patients with higher levels of need.
Provides information on and assists with enrollment in social service programs and state and federal regulations specific to the community.
Employs social determinants of health screenings to develops a plan that will address the needs identified.
Works with the patient, family, community, state and federal agencies, inpatient facilities and any other applicable resources, to formulate a plan for longitudinal support and self-management of social needs.
Gathers information regarding the patient's social needs, family support system, financial resources, and available community and governmental resources.

Qualifications:
Master's degree in social work (MSW) from an accredited graduate school of Social Work required.
LMSW, LSW or LCSW strongly preferred.
One to two years of direct experience with arranging community services for complex populations
Bilingual language skills are a plus.
Whether a position can be supported for remote work will be assessed based on whether ChristianaCare is able to meet the business requirements of the proposed remote location.
