ChristianaCare's CareVio team is seeking a Senior Social Worker I to join our team. This individual willwork as part of a dynamic team working with Christiana Care Union Campus as a part of the readmission team working with patients in the post-acute period to prevent readmissions.  This position will assist members and families to identify social, emotional, and environmental needs to connect them with available resources/ services through the provision of a full spectrum of discharge planning and social services.   This position is located in the CareVio Virtual HUB at Ave North, with hours Monday-Friday 8-4:30 with an occasional evening requirement. 

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Perform ongoing telephonic case management activities of assessment, problem identification, planning, implementation, coordination, monitoring, and evaluation of case managed members
  • Collaborates with the members/family, physicians, staff, and other health care professionals across the continuum to provide care coordination and provide the member services throughout the continuum of care
  • Identifies problems or gaps in community resources that impact outcomes and takes leadership in efforts to effect changes. Coaches members on red flags, triggers, medication management, and followup appointments
  • Gathers and assesses information regarding the member's physical needs, mental status, family support system, financial resources, and available community and governmental resources to support care plan development
  • Provides psychosocial support through individual or caregiver option counseling. Continuously reviews service area for group support needs and opportunities and refers to appropriate resources
  • Manages assigned case load in an efficient and effective manner utilizing time management skills to facilitate the total work process directly monitoring assigned members
  • Confers with Medical Directors regarding complex cases requiring physician input regarding the treatment plan or physician to treating physician collaboration
  • Adheres to organizational and departmental policies and procedures and credentialed compliance
  • Collaborates with other members of the care management team to achieve case management, clinical, operation and financial outcomes

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Masters degree in Social Work from an accredited graduate school of Social Work.
  • Minimum of 2 years' experience related work experience in hospital discharge planning, health plans and or home care preferred.
  • MD licensure required; LCSW
  • Must complete Case Management Certification within 18 months of eligibility (working under CCM for 1 year)
  • Training time on the job is three to six months.
