The Kidney Transplant Program offers the only adult kidney transplant program in Delaware. More than 1,000 patients rely on our transplant team for kidney failure care. The care provided encompasses kidney transplant evaluation, waitlist care, after transplant care and transplant education, as well as living donation. The team is multidisciplinary in nature, including transplant surgeons, transplant nephrologists, nurses, social workers, dietitians, pharmacists, financial coordinators, medical assistants and office support.

What you will be responsible for?

  • Lead an effective transplant quality assurance and performance improvement (QAPI) program that is multi-disciplinary in nature, data driven and covers all phases of transplant and donation in a continuous cycle of review and improvement while addressing patient safety, clinical care, quality of life, and those services provided under contract or arrangement. Ensures that the program utilizes defined methods for conducting analyses, developing and implementing corrective actions, monitoring improvements, and assessing the efficiency and sustainability of actions taken.
  • Maintains currency with transplant regulations for all agencies and ensure health system leadership, transplant program leadership and staff are kept apprised of changes in regulation and that practices are aligned with regulatory requirements.
  • Develops, monitors, and supervises clinical indicators as part of the overall quality improvement program for the transplant program in collaboration with the physician leaders, clinical manager and administrative director. Organizes and facilitates multi-disciplinary teams to successfully implement performance improvement initiatives to achieve program quality improvement goals. Annually coordinates a review of the PI indicators defined and makes recommendations for revisions.
  • Coordinates scheduling of announced visits/surveys. Coordinates preparation of UNOS reports including annual personnel report, changes in key personnel, and reports requested by the Membership and Professional Standards Committee. Monitors and supervises compliance with UNOS requirements for data submission. Alerts the management team if there is not full compliance with the requirements. Conducts an analysis of quality programs and patient care outcomes and ensures these are included in the development of organizational strategic plans.
  • Supports the ongoing education and development of staff to advance individual, departmental, divisional and organization goals.
  • In collaboration with the clinical manager, prepares required documents and reports as the need arises by CMS and UNOS regulations.
  • Participate in regional and national committees and organizations with activities relating to transplant care .

Education and Certification

  • Registered Nurse required. BSN required; MSN preferred.
  • Minimum of 5 years clinical and 1-3 years of healthcare management and leadership experience including a quality assurance background preferred. Clinical experience in transplantation or critical care preferred.
  • Certification of either CPHQ (preferred) or CCTC within 2 years in the role.
