The digital RN cares for patients virtually within the scope of the nursing practice license.  Leveraging a variety of technology tools and software, the digital RN is responsible for the clinical monitoring and care for the patient including, but not limited to:  clinical assessment, evaluation of clinical information that is being monitored and collected from patients, managing clinical protocols, assessing psychosocial needs, recommending services for the patient to the provider (i.e home health, DME, rehab services), creating collaborative care plans, performing case management functions and escalating clinical concerns to a provider in accordance with established protocols.

***Please note the hours of operation will be 7:00am -8:00pm, with a weekend rotation required.  There will be 2 shifts:  7:00am-3:30pm and 11:30am to 8:00pm***


  • Demonstrates knowledge and skills necessary to provide care appropriate to pediatric and adult patients, including the ability to obtain and interpret information to identify patient needs and care needed

  • Collaborates with team members to provide a smooth, organized and efficient flow of patients in a virtual telehealth platform environment

  • Collaborates with the Patient Digital Ambassador (PDA) to monitor clinical dashboards and real-time clinical biometrics

  • Performs case management functions including, but not limited to:  assessing for clinical and psychosocial needs, identifying the need for home healthcare, rehab services (PT, OT, Speech, ect), assisting with financial applications, including Medicaid, identifying and assisting with the need for guardianship.

  • Ensures timely setup of ordered services - home health, DME, rehab services

  • Utilizing a camera, engages with patients and/or designated medical spokesperson

  • Performs Transitional Care Management Interactive Calls

  • Helps patients and families to navigate the health system to receive necessary care

  • Facilitates communication amongst providers, including scheduling huddles to discuss patient situations when necessary

  • Responds to patient crises situations (triage and referral to Emergency Department)

  • Provides Care Coordination and Care management as appropriate

  • Provides patient education and wellness counseling

  • Performs medication reconciliation

  • Performs virtual Nurse Visits by protocol

  • Supervises the clinical work of Patient Digital Assistants

  • Assists providers performing co-visits

  • Closes gaps in care

  • Functions as a liaison between patients, physicians and staff to provide accurate communication

  • Performs assigned work safely, adhering to established departmental safety rules and practices;  reports to practice manager , in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.

  • Performs other related duties as required.


  • Graduate of an accredited school of Nursing; BSN within 3 years of hire

  • RN license or eligible for licensure in the state Delaware/Maryland, New Jersey and Pennsylvania

  • Two years' experience as an RN, primary care experience preferred

  • Certification in Case Management through the Commission for Case Management (CCM) within 18 months of eligibility is required.


  • Expert ability to leverage a variety of technology platforms and software

  • Ability to rapidly adjust to change

  • Clinically astute with the ability to interpret clinical biometrics and recognize the need for intervention

  • Ability to effectively practice utilizing approved clinical protocols

  • Knowledge of case management and care coordination professional standards and scope of practice

  • Exceptional ability to work collaboratively in a team-based environment

  • Exceptional ability to engage virtually with patients, including use of a camera

  • Knowledge of Quality concepts, measures, goals, and outcomes

  • Knowledge of current nursing practice for adult and geriatric patients

  • Knowledge to take and record vital signs (in a virtual environment)

  • Knowledge of medical terminology

  • Knowledge of managed care insurance

  • Knowledge of ICD 9/10 and CPT codes

  • Knowledge of referral process

  • Ability to use virtual telehealth visit technology platform

  • Ability to organize and complete various tasks in a virtual telehealth environment simultaneously

  • Ability to display excellent customer service skills

  • Ability to apply good oral and written communication skills

  • Ability to respond to medical and non-medical emergency situations

  • Knowledge of computer software

  • Knowledge of physician office systems and practices

  • Knowledge of the patient portal


Current RN state licensure in DE/MD, PA and NJ
